QA Templates and Forms

A recommended format for provider's procedure documents. It is not compulsory to use this Procedure template, but it does illustrate the kind of detail that will be required in your procedural documents.

If a provider intends to deliver programmes of 3 months duration or more on a commercial and profit making basis, it is required that a policy and procedures for the Protection of Learners (PfL) are put in place to protect learners should the programme cease unexpectedly. The provider should complete this PfL compliance form and submit it with their application along with the evidence of the arrangements the provider has put in place to protect learners.

Where a provider consists of two or more constituent centres (fixed locations of learning) use this form to supply details of each.

Self-Evaluation of Programmes and Services

Under the terms and conditions of registration with FETAC providers must carry out an evaluation of at least one of its programmes within a calendar year of the date of its initial registration and at least once every five years thereafter. This evaluation process is a critical component to ensure the effective application of quality assurance policies and procedures of providers.

Providers who have been registered with FETAC for more than one year should now ensure completion and submission of their self-evaluation report in accordance with this requirement. A letter to remind those providers due to submit will be sent out by FETAC.

The self evaluation reports provide a key piece of evidence for monitoring of providers by FETAC.
